Waist-to-hip ratio, usually shortened to WHR, describes where your body stores fat rather than how much of it you carry. It compares the circumference of your waist against the circumference of your hips, and the World Health Organization treats it as a screening measure for metabolic risk. The reasoning is physiological: visceral fat packed around the liver, pancreas, and intestines behaves differently from subcutaneous fat sitting on your hips and thighs. Abdominal fat is metabolically active, releasing fatty acids and inflammatory signals straight into the portal vein, and it tracks with insulin resistance, high triglycerides, and cardiovascular disease more tightly than total body weight does. Two people can share the same weight and height, and therefore the same BMI, while one carries a far higher risk profile. WHR is the cheapest way to spot that difference, and all it takes is a tape measure. The arithmetic is one division: WHR = waist circumference ÷ hip circumference, using the same unit for both. Because it is a ratio, centimetres and inches give an identical answer as long as you do not mix them. Measurement technique is where the result is won or lost. Measure your waist at its narrowest point, typically just above the navel, standing relaxed and breathing out normally, with the tape snug against skin but not biting into it. Measure your hips at the widest point around the buttocks, feet together, tape level all the way round. The WHO then reads the result against sex-specific thresholds. For women: under 0.80 is low risk, 0.80 to 0.85 moderate, above 0.85 high. For men: under 0.90 low, 0.90 to 1.00 moderate, above 1.00 high. The bands differ because oestrogen drives fat storage toward the hips in women, so the same 0.88 means different things in each body. Here is the calculation on real measurements. A woman measures 72 cm at the waist and 98 cm at the hips. 72 ÷ 98 = 0.734, which rounds to 0.73 and sits comfortably in the low-risk band, since it is well under the 0.80 threshold. A man measures 95 cm at the waist and 100 cm at the hips. 95 ÷ 100 = 0.95, landing between 0.90 and 1.00 in the moderate band, a signal worth acting on rather than ignoring. Note what happens if he loses 7 cm from his waist: 88 ÷ 100 = 0.88, back under the 0.90 line, even though his hip measurement never changed. That is the practical value of the ratio; the waist number is the one you can move. Specific situations where WHR earns its place. A 46-year-old office worker with a BMI of 24 and no obvious weight problem measures 0.94 and takes the abdominal fat seriously before a routine blood panel flags anything. A woman tracking a 12-week walking plan sees her weight barely shift while her WHR falls from 0.86 to 0.81, confirming her body composition improved even though the scale lied to her. A company health screening records WHR alongside blood pressure for 200 staff, because a tape measure costs nothing and needs no calibration. A rugby prop with heavy leg musculature and a 0.91 ratio compares it against a DEXA scan and finds the number is skewed by muscular hips rather than abdominal fat. The pitfall that trips almost everyone is measuring the waist at the belt line instead of the narrowest point, or sucking the stomach in. A 3 cm error at the waist shifts the ratio by roughly 0.03, enough to move you across a risk band. Measure twice, and measure at the same time of day, since a heavy meal or bloating can add a centimetre or two. WHR also has real blind spots: it says nothing about total body fat, fitness, or muscle, so a very muscular person or someone with unusually broad or narrow hips can get a misleading reading, and it is not designed for pregnancy or children. Waist circumference on its own, above 88 cm for women or 102 cm for men, is arguably just as informative and easier to measure consistently. Use WHR alongside BMI, waist size, blood pressure, and blood work rather than in isolation.
